MPH School Safety and Reporting - Student Statement Form

The safety and security of students, staff, and our community are a top priority of Mary Putnam Henck Intermediate School and Rim of the World Unified School District. As such, please review relevant information about school safety, PBIS, and the importance of reporting concerns in accordance with California state-mandated reporting requirements.

If you are experiencing an emergency please call 911, and follow up with school personnel for assistance as relevant to the circumstance/ situation. 

Students may utilize the school physical or digital student statement form to report concerns involving other students. If a student or parent/guardian needs to report a concern involving a staff member please contact a school administrator directly to report a concern. Student statement forms are for student-related concerns. 

Physical statement forms are available throughout campus, in classrooms, offices, security, and by request from any school employee. The physical statement form may also be printed by accessing the document below. 

Digital statement forms may be submitted by accessing the below link. A student must sign in to their district-provided Google account to access, complete, and submit the digital student statement form. This form is also available in the MPH the MPH student bookmarks are accessible when a student is logged into their district Google account.
